| The Health Partners Pharmacy and Therapeutics (P&T) Committee has reviewed and approved the Formulary for 2008. Use this link to view, print or download the Formulary. You can also call our Provider Services Helpline at 215-991-4350 to request a hard copy.
Please call the Pharmacy department at 215-991-4300 with questions/feedback. If you have suggestions for additions to the Formulary, please submit these requests in writing to the Pharmacy department. Requests for additions to the Formulary will be reviewed by the P&T Committee. Health Partners will continue to update the Formulary quarterly and communicate the changes to you in a timely manner.
If you are prescribing for a specific member, and wish to request a non-Formulary drug, or a Formulary drug that requires prior authorization, please first review this list of Drug-Specific Prior Authorization Forms. Using one of these forms will help assure that all necessary information is provided for the drug being requested.
To request a medication that does not have an associated Drug-Specific form, please complete the Non-Formulary Drug Prior Authorization Form.
Send completed Prior Auth requests to Health Partners' Pharmacy department using the Fax numbers shown on the form.
