Health Partners Foundation Partners Award
Nafatari Manigualt and Yvette Dickenson, Project GRAD’s PA College Access Challenge Grant Program at Chester High School
Selected for their leadership of a Project GRAD PA Challenge program that provides students and their families with mentoring and tutoring in preparation for college and careers. Their combined efforts doubled the number of Chester High School graduates who started college this year, with the help of more than $800,000 in academic scholarships |

Elizabeth “Lily” Ramos, WB Saul Agricultural School
We are recognizing Elizabeth for her eco-friendly efforts as a Teens 4 Good intern at the Federation of Neighborhood Centers, a non-profit that hires inner city youth to transform vacant lots into community farms. |
Health Partners Making a Difference Award
New Kensington Community Development Corporation (NKCDC)
Founded in 1985, New Kensington Community Development Corporation strengthens the physical, social, and economic fabric of the Kensington community through neighborhood revitalization, anti-violence initiatives, education and healthcare services. |
Turning Points for Children
Committed to support and services to children and families in need, Turning Points for Children provides after-school programs, teen parenting education, and in-home services for at-risk children who have been abused or neglected.

Founded in 1999, Health Partners Foundation (HPF) works to fulfill unmet needs in disadvantaged communities of the Philadelphia region. The Foundation is primarily funded by Health Partners employees with the help of supportive vendors and other individuals. Here is how we help make a difference:

Making A Difference Awards
|
|
|
Each October, HPF hosts an awards ceremony to honor the region’s outstanding individuals and organizations that are working hard to make a difference in their communities. Past awardees include: Federation of Neighborhood Centers, Philadelphia Chinatown Development Corporation and Lisa Nutter, president of Philadelphia Academies. |

Make Dreams Reality Scholarships
|
|
|
High achieving low-income students who want to attend college or technical school are offered a Make Dreams Reality Scholarship. These scholarships help bridge the gap between available funding for their education and related expenses.
|
|
|
Be There! Perfect Attendance
|
|
|
This program encourages students to attend school daily as an important step toward academic and life success. The Achiever's program awards certificates and other honors to 9th and 10th grade students who maintain perfect attendance for at least three of four grading periods in our partner schools.

|
Healthy Advances
|
| |
Inspiring science careers among urban youth, Healthy Advances recognizes a student in grades 4-6 for their best health-related science project, at the annual George Washington Carver Science Fair. Cash awards go to the winning student and his or her school. |

Future Leaders Academy
|
|
|
Non-traditional learning experiences are offered in the Future Leaders Academy, enabling students to gain leadership skills through one-on-one and group mentoring. The program helps students develop skills in cultural competency, budgeting, critical thinking, research, public speaking, resume writing and job interviewing. An in-depth debate workshop culminates in a special live presentation called The Great Debate. |

Thanksgiving Food Baskets
|
|
|
HPF helps those who might otherwise not enjoy a Thanksgiving meal by contributing to food baskets that are donated by Health Partners employees. |
|
|
Holiday Gifts
|
|
|
To help spread cheer during the holiday season, HPF supplements gifts donated by Health Partners employees. The Foundation also offers financial support to other organizations providing holiday gifts. |
|
|
Summer Heat Emergency Relief
 | |
Through its Chill program, HPF provides fans to shut-ins, children with special needs and others who require relief from the summer heat. |

Supporting Committed Organizations

Many other foundations support underserved communities in Philadelphia and HPF helps them succeed even more through small donations. Organizations that have benefited from our support include: Mothers in Charge, the African-American Children's Book Fair, Teen Shop, Evoluer, Rubye's Kids and many more.
